I create I form & in this form I have 1 lov & I want use this lov for 4 items. Is this possible. if yes then please guide me how can I do it.

Well, it is possible to use one Record Group as a source for several List of Values. Though, as you have to map LoV columns with form items, you'd better write 4 different LoVs.
